Aadhar Card Form

Aadhaar refers to a unique 12-digit identification number assigned to every resident of India by the Government, basis their demographic and biometric information. As no two individuals have the same demographic and biometric details, it ensures the uniqueness of every Aadhaar Number. It is considered to be a proof of residence as opposed to the popular opinion of it being a proof of citizenship. Also, it is accepted as a residential proof only within the geographical boundaries of India and is a not a valid identification proof for Indians traveling abroad.

This is collected by the statutory authority called UIDAI (Unique Identification Authority of India). It was formed by the Government of India in January 2009, especially for the purpose of generating the 12-digit identification number for every Indian resident. UIDAI operates under the jurisdiction of the Ministry of Electronics and Information Technology, according to the provisions of the Aadhaar (Targeted Delivery of Financial and other Subsidies, benefits and services) Act, 2016. Though an Aadhaar Card establishes identity and is a proof of residence within India, enrolment under it is not compulsory. However, individuals opting for it have to follow a streamlined verification process that follows a few steps. The process can be initiated by booking an appointment after which a list of essential documents need to be submitted. These documents should not misrepresent or suppress true information to prevent unwanted complications later. However, in case of errors while filling the form, the applicant can revisit the form to rectify the errors.

Aadhar Card Enrollment Form

The Aadhaar Card Form helps residents of India to apply for an Aadhaar Card. The form has been made available both online and offline to enable individuals to access it according to their convenience. For filling up the form offline, applicants can visit the nearest Aadhaar Centre, while those who prefer the online mode can download it free of cost from UIDAI’s website. The Aadhaar Card Form performs the dual role of enrolment and correction of information in the existing Aadhaar Card. The applicant has to fill the form with correct information and submit proofs for all details provided. The Aadhaar Card Form comprises of 11 basic fields. However, filling up all of these fields are not mandatory. Those fields that are necessary are marked with asterisks. At the end, the form has to be duly signed by the applicant. The fields are:

  • The acknowledgement number or the pre-enrolment ID received by the applicant after applying for Aadhaar Card online
  • Submit the Tax Identification Number (TIN), if the applicant has one, or National Population Register (NPR) survey slip
  • Full name of the applicant
  • Gender, that is, male or female or transgender
  • Age or Date of Birth
  • Residential address of the applicant as mentioned in the address proof document.
  • Details regarding the family of the applicant like that of parents as well as spouse and children (if applicable), etc.
  • The option to allow or deny UIDAI the right to share these details with different agencies
  • Bank details of the applicant (optional)
  • In case of the inability to submit the necessary proof of identity and residential address, mentioning the credentials of the HUF (Hindu Undivided Family) or introducer is mandatory
  • Signature or thumbprint of the applicant, which is a mandatory field

Steps to Fill an Aadhaar Card Enrolment Form

You need to follow these steps for enrolment:

Step 1: Visit UIDAI’s official website and download the Aadhaar Enrolment Form

Step 2: Fill the details as per the attached document proofs

Step 3: If case of the applicant being an infant, that is, an individual below the age of 5 years, either of his/her parents has to submit the Aadhaar Card Form

Step 4: Mention the email address and contact number

Step 5: The contact number is automatically linked to Aadhaar

Step 6: Click on the ‘Submit’ button

Step 7: If there are errors after submission, it can be rectified within 96 hours for free. Rectifications made after this deadline is liable to additional charges that are payable to the government.
