The EPF Form 5 or Employee Provident Fund Form 5 is a form which officially states that a particular employee has been enrolled for the Employee Provident Fund Scheme. The Form 5 provides details about new joinees/employees who are eligible for EPF/EPS/EDLI and intend to open an EPF account.
All organizations with an employee strength of 20 and more (in the formal sector) are required to provide EPF facility to all its employees.
EPF Form 5 is mentioned in Para 36(2) of The Employees’ Provident Fund Scheme, 1952. The form enables organizations as well as the EPFO to register the number of people who have joined the scheme for the first time. Once an employee has been successfully enrolled, they will be allotted a Universal Account Number, under which, the employee’s as well the employer’s contributions start getting deposited.
The Employee Provident Fund Organization ensures that all companies within its jurisdiction are active participants of the EPF scheme. It is mandatory for the employer to provide the EPF facility to all its employees.
