• Your family must be named under the Yojana
• Annual income less than 4 lakhs
• You should be a U-win cardholder
• All urban and rural (ASHAs) activists, class 3 & 4 state government employees & reporters
• Senior citizens of families having annual income of maximum of 6 lakhs

• Go to the kiosk at your respective Taluka or area’s civic centre
• You can even visit local mobile kiosks
• The families falling under BPL having an extra income that’s less Rs. 4 lakhs are verified at the enrolment time
• Biometrics like fingerprints, photos & iris scans are taken.
• Application form needs to be filled in Gujarati
• Required documents need to be submitted.
• Verification is done
• Post verification, the person receives the MA Amrutum card

Making a claim under Mukhyamantri Amrutum Yojana is a completely cashless process. Once the emergency has been identified, the patient is required to submit their QR coded card (Quick Response Coded Card) to the network hospital for approval. Post approval, the hospital will go ahead with the treatment.
